By booking an appointment I fully accept the risk of my services and understand that this procedure promises no guaranteed results. I understand factors including but not limited to medication, minerals, hormones, pregnancy/breastfeeding, hair history, health conditions and diet may affect my results. I understand that my professional will take every precaution to minimize or eliminate negative reactions or outcomes to the best of their ability. I assume any risk and take full responsibility and waive any claims of personal injury, death or damage to my person and personal property associated with my services including but not limited to temporary or permanent damage to my hair or unsatisfactory results from the services provided to me. I have accepted this agreement freely under no promise or guarantee. I will consult (Hairxchloehatfield) directly and immediately should I have any complications after receiving my services. I have been given the opportunity to ask my professional questions and they have been answered to my satisfaction. For future services, I agree to inform my service professional of any changes in my medical status and/or the above information. C O P Y R I G H T I would like to grant Hairxchloehatfield, permission to publish and reproduce photographs and videos of me, my face, my hair, lashes, nails, etc. both during and after this procedure, for any advertising, education, or other purposes, including the right to retouch these photographs as deemed necessary by them. I further expressly assign any copyright in these photographs to (Hairxchloehatfield). P O L I C I E S + P R O C E D U R E S L A T E P O L I C Y We totally get that life happens- but to respect everyone’s time (yours, ours, and our next guests), please try to arrive on time for you appointment. If you arrive more than 15 minutes late, we may need to reschedule your appointment to ensure we have enough time to give you our best work. If we’re still able to accommodate you, a $30 late fee will be added to your service total. Thanks for understanding and helping us keep things running smoothly. C A N C E L L A T I O N P O L I C Y We know things come up, and we’re all about flexibility, but we kindly ask for at least 24 hours notice if you need to cancel or reschedule your appointment. Cancellations or no- shows with less than 24 hour notice will have the full service charged to the card on file. Frequent cancellations may require a deposit in order to book for the future. After a no-show appt you may be required to pay a non-refundable deposit for future bookings. P A Y M E N T P O L I C Y All payments are due at the time of service. We accept card only and can use the card on file. Prices may vary based on hair length, density, and service complexity. We will be quoting your service for you in your consultation. If you have other questions - please ask. All sales are final, and services are non-refundable once completed. If you have any concerns, please let us know within 7 days- we’re always here to make it right. S A L O N P O L I C Y To protect the safety of our clients and stylists- and keep our salon a relaxing environment -we kindly ask that you follow these guidelines: -No kids at appointments unless they are receiving a service.(nursing babies are okay!) -No extra guests unless they are booked for a service -Kids with appointments must be accompanied by one adult only-1 kid = 1 adult -No other children should be brought to the salon -We are not responsible for any damaged clothing during your service. Please avoid wearing anything you’d hate to risk- color happens!
